Signal Anomaly Causes Fatal Train Accident in Cicalengka: KNKT

Inp.polri.go.id - Jakarta.  The final report from the National Transportation Safety Committee (KNKT) investigation team found that an anomaly in the form of an un-commanded signal had caused the recent fatal train accident in Cicalengka, West Java. "KNKT concludes that this accident occurred due to a signal sent by the interface system without instructions from the mechanical block signaling equipment," said Acting Head of the KNKT Train Accident Investigation Sub-Committee Gusnaedi Rachmanas during a press conference on Friday (16/2/2024). He explained that there was a difference in block systems between Haurpugur Station and Cicalengka Station. The Haurpugur Station uses an electrical block system, while the Cicalengka Station uses a mechanical block system, antaranews.com reported. To connect these systems, Gusdaedi said, an interface device was used at the Cicalengka Station. The uncommanded signal occurred due to a high-amplitude voltage surge experienced by the Cicalengka Station interface when receiving signals from the Haurpugur Station. At that moment, the Haurpugur Station sent a departure signal for Train 350 CL Bandung Raya to the Cicalengka Station. Upon receiving the departure signal, a disruption occurred in the Cicalengka Station interface, ultimately resulting in the transmission of an un-commanded signal indicating that the track section towards the Haurpugur Station was safe for the train to pass. "This indication of safety towards the Haurpugur Station became the reference for the station train dispatcher at the Cicalengka Station to allow Train 65A Turangga to proceed directly towards the Haurpugur Station," Gusnaedi said. In addition to the signal anomaly, Edi also revealed the presence of confirmation bias as a contributing factor to the accident. Confirmation bias refers to the belief of station train dispatcher in the block system, leading to a lack of confirmation between the train dispatchers of the Cicalengka Station and the Haurpugur Station when dispatching trains from each station. The train accident on the track between the Cicalengka Station and the Haurpugur Station on 5 January 2024 had claimed the lives of 4 passengers and injured  37 others . (mg/inp/pr/nm)
